* If the dates of the appointment change by one month or less, change the dates on the approved appointment form and obtain the Chair's signature. If the salary decreases or there is a change in basis, provide documentation indicating that the faculty member agrees to or requests the change.

*** If the appointee has worked in the Medical School in the last six months in a research staff or research faculty position, this is not needed.
