Requesting an Appointment
We are glad that you have chosen to seek an opinion from a physician at Northwestern Rheumatology!
Before your visit can be scheduled, we need some information from the doctor who is sending you to us (usually your regular primary care doctor) so that we may best serve you.
We need all of the following:
- A referral form, if your insurer requires one.
- A statement of the specific reason for referral, or of the clinical issue upon which the referring doctor would like us to comment (this can be included in the referral paperwork).
- A copy of the doctor's notes from your most recent visit (and any previous visits that are directly related to the problem for which you are coming to us).
- Copies of any recent lab testing (blood, urine, etc.) and any relevant medical imaging (X-ray, MRI, cat scan, etc.). The written reports are sufficient, but copies of the actual images either on film or on CD-ROM are even better.
In order to release your medical records to Northwestern, you will need to submit a request in writing to the facility where the testing was done. Most facilities accept our one-page form for this purpose; it can be downloaded from the link at the bottom of this page.
