Patient safety rounds

Patient safety rounds are designed to identify potential problems or errors in the system, and discuss possible solutions (including technological changes, system wide changes in protocols, improved communication, etc). Jane Wilcox, MD, is a third year resident who recently had the opportunity to participate on a panel discussion on making hospital discharges safer.
"This experience reiterated that discharge planning requires a team approach involving nurses, physicians, pharmacists, patients and families. Many researchers are now studying the post-discharge “space” and designing mechanisms to improve communication and decrease adverse events "
